Overview

Fidelis Care is awarding grants to healthcare providers and community-based organizations in New York State to help support maternal health and wellness programs.

This year’s program is focused on funding innovative strategies in postpartum care and maternal mental health. Please ensure your application connects to these goals.

Eligibility

• If your office or organization runs a program that helps supports postpartum care and maternal mental health, particularly for high-need, low-income women and their families, then Fidelis Care encourages you to apply for the grant. 
• Providers or community organizations must serve New York State.

Benefits

• Grants up to $20,000 will be awarded

How to Apply

• Complete the Fidelis Care Maternal Health Program Grant Application Form.
• Save the form to your computer.
• Email the completed form to Fidelis Care’s Health Equity team.

Please Note: Applicants must answer all of the questions on the form to be considered. Some reporting is required. Awards will be announced in August.
